The Health and Care Act includes a legal duty for decision-makers across NHS bodies (Foundation Trusts, NHS Trusts, Integrated Care Boards and NHS England) to collectively consider the impact of their decisions on the health and wellbeing of everyone, the quality of healthcare services and the sustainable use of NHS resources.

Stewardship and value-based healthcare provides an opportunity to support health and care systems to meet their Triple Aim duty. This approach empowers systems (and organisations) to deliver the mindset shift, culture and skills required to make healthcare sustainable.

Arden & GEM has extensive experience of supporting places and systems to explore how to develop a value-based approach to delivering health and care services. Our support is unique in bringing together the importance of cultural change, a focus on populations as well as using data to drive improved population outcomes. We have also developed a tool to look at allocative efficiency and value within specific pathways such as MSK, knee pain and stroke.

Our Stewardship and Value-based Healthcare support provides the experience and expertise required achieve the best possible outcomes for individuals and populations while optimising resource utilisation. Our support follows three key stages:

 

Our workshops are specifically designed to fit with the needs and objectives of the organisation or health system. Each programme is developed to enable NHS professionals to deliver a culture of stewardship via five core tasks:

  • Thinking in populations
  • Designing in populations
  • Working in populations
  • Leading for populations
  • Optimising personal value.

Workshops bring together stewards from systems to understand their envisaged culture and responsibilities. Stewards represent those groups with a responsibility for resource allocation including frontline clinicians from primary, acute and community care, and finance leads.

The sessions are tailored to meet the system’s needs while addressing the core issues. Support is provided to map what is delivered in their system and the cost attached to delivery.

Our experts will assist systems to highlight agreed areas of focus for further workshops where wider stakeholder agreement is needed. We actively seek areas where systems can stabilise, reduce or increase activity dependent upon the value it delivers for patients. Tailored improvement projects will be launched to take forward such as cost improvement initiatives.

By linking key initiatives such as population health management (PHM), clinical stewardship, and a renewed focus on outcomes, our expert support is helping Integrated Care Systems (ICSs) and Partnerships to develop their capability to deliver the Triple Aim.

We have worked with ICSs in the NHS Midlands, East of England, and North East and Yorkshire regions to deliver a culture of stewardship and enable equitable care delivery.

Health and care systems have taken forward improvement projects, including cost improvement initiatives, and received continued support in monitoring agreed outcomes through development of new dashboards.

Key outputs have included establishing stewardship groups, development of value frameworks and outcomes measures, and the creation of bespoke dashboards to understand delivery and analyse resource usage across the system.
